Make a Referral

A referral is the highest compliment a therapy practice can receive.  We appreciate and welcome referrals from outside agencies, hospitals, clinics, schools and other community resources.  As soon as we receive a referral we will begin processing of coordinating services and verifying insurance coverage.  If a client has "inactive" insurance, we will not be able to schedule an intake until we have confirmed insurance is active or verified client will be self-paying for services.  We will work with you, the referral source, to establish services and meet the needs of the client.  We appreciate the support and assistance in coordination.  If you have any issues with submitting this form or need additional assistance please email us at charline@northcountrycounseling.net.
